Checklist
Please ensure that the following items
are completed prior to application submission:
1. All use of Port
Authority property (including submerged lands and disposal
areas) or crossing/adjacent to Port Authority or PTRA
rail lines MUST BE REDLINED.
2. Application and
drawings may be submitted online to gmcmahan@poha.com. If mailed, they must be sorted and stapled in order,
2 copies, as follows:

Drawings -Plan, Profile (Elevation), and an overview
of the area at a scale relative to the length of the pipeline. |

All coordinates must be in NAD 1983 StatePlane
Texas South Central
FIPS 4204 (Feet). |
| b) |
Elevations must be referenced to Mean Low
Tide datum for water crossings. |
| c) |
Include all inflection/turning points along
the pipeline with coordinates or distance
& direction for each point |
| d) |
If GPS was used to obtain coordinates,
include the accuracy level of the collection
device and whether the data was differentially
corrected. |
| e) |
Designate individual pipelines on separate
maps. |
| f) |
Electronic data formats (in order of preference) |

3. Drawings should
be 81/2" x 11", 81/2" x 14", or
11" x 17" folded to fit into an 8 1/2"
x 11" file folder.
4. If milepost is
referenced, give the associated distance from known
street, road or highway.

Proof of insurance is verified by the PHA at ins-cert.com. Your agent or broker must register at http://ins-cert.com/ and enter your policy data.
Payments (fees
for applications, license, renewals, and as-built deposits)
should be submitted with a copy of the first page of
